You are required to present this at each visit. Acceptable forms include: valid drivers license, valid state id, military id or valid passport from country of origin.
Medicare Prescription Drug Plan:
Please bring your Prescription Drug Plan with you if you have Medicare.

Laboratory and X-Ray Test Results:
Lab results vary in result reading time and may not be released until reviewed by your doctor.
